PERS Checklist - "What do I do if I want to retire?"
As soon as you schedule your PERS session, also contact the benefits section of Human Resources at 346-2967 to set up an appointment as a follow-up to your One Year Group Counseling session. This meeting allows you to get information about retiree insurances, Medicare, Social Security, University services and other retirement issues.
If you participated in the PERS/TIAA-CREF Retirement Plan, you also need to contact TIAA-CREF at 1-800-842-7715 for retirement forms.

If you are eligible and plan to start drawing Social Security, you should also apply for benefits in person at your local Social Security office. Before you go, Social Security prefers that make an appointment by calling toll free 1-800-772-1213. You can apply by telephone or on-line, but we recommend that you visit the local office. File for Social Security at least three months before you plan to start drawing benefits. Please review the Social Security Website for a list of documents you must provide when you apply for benefits.

If ONE item of this group is furnished showing date of birth, no further evidence of age is needed. Any ONE (1) of these:
TWO items of this group from different sources are sufficient if age or date of birth is shown. Example: One child's birth certificate and one driver's license. Any TWO (2) of these:
